physical findings and abnormal laboratory tests) should receive the benefits of
HGH replacement.
Q: Is HGH (Human Growth Hormone) legal?
A: Yes. HGH is an FDA-approved medication for certain medical disorder,
including Adult GH deficiency. As long as the patient has the symptoms, physical
findings and laboratory tests proving the diminished GH production, GH is
indicated.
Q: Is Testosterone an illegal steroid?
A: No. Testosterone is an FDA-approved medication for males and females with
proven testosterone deficiency. Although it is a steroid medication its indications
are not the same as the illegal steroid drugs.
Q: Which products does Dr. Maiquez use?
A: For HGH we use the Omnitrope Sandoz pen, because of the convenience of a
simple pen that does not require pre-mixing. Also this pen can be re-used for at
least 2 years, replacing only the medication cartridge, which make this pen very
cost-effective.For in-office Testosterone injections we use Testosterone
Enanthate or Cypionate. For Testosterone cream, Estrogens, Progesterone,
Cortisol and Thyroid hormones we recommend bio-identical forms from a
compounding pharmacy.
Q: What does "bio-identical hormones" mean?
A: Most hormones used to prevent or treat deficiencies can be classified as
"synthetic" when their chemical structure resembles (but does not match) the
structure of the human hormone. They are "bio-identical" when, despite being
manufactured in a lab, they match the exact chemical structure of their human
corresponding hormone. Bio-identical hormones are safer and better tolerated
than the synthetic ones.
Q: Does Testosterone Replacement Therapy causes Prostate Cancer?
A: No. Testosterone is important for the normal health of the prostate gland.
Testosterone used to be linked to prostate cancer but the scientific literature is
demonstrating the opposite in recent years. Actually, a few research have linked
low testosterone to prostate cancer. (suggested reading: "Testosterone for Life"
by Abraham Morgentaler, M.D.)

A: Aesthetic procedures are performed entirely by Dr. Maiquez. These include
Botox, Mesotherapy, Dermal Fillers (Restylane, Juvederm, Autologous Fat

testosterone or other hormones very expensive?
A: HGH treatments used to be thousands of dollars a month. Physician-supervised
HGH replacement is nowadays about $ 150 - $ 300 dollars a month. A Physician-
supervised program guarantees appropriate medication dosage, quality and
potency.
Testosterone replacement therapies are about $20- $25 dollars a week or $40-
$50 dollars every two weeks when administered in the medical office.
Q: Do Health Insurance Plans cover or pay for office visits for Anti-Aging

A: Most plans do not cover for anti-aging physician office visits.
Q: Do Health Insurance Plans cover or pay for HGH?
A: Most Health Insurance plans do not cover or pay for HGH treatments.
Q: Do Health Insurance Plans cover or pay for Testosterone, estrogen,
progesterone, thyroid or cortisol treatments?
A: Most Health plans do cover or pay for synthetic testosterone, estrogen,
progesterone, thyroid and cortisol treatments. The best option for replacement of
these hormones is the bio-identical hormones and most insurance plans do not
pay for those, except some cases of testosterone and cortisol. The insurance
plans that pay for injectable testosterone usually require pay for the medication
for self-injection at home.
Q: How much does your Anti-Aging Program Cost?
A: Initial Consultation fee is $ 150.00. First Follow-Up Consultation to discuss
laboratory results is free ($90.00 value). Follow-Up Office Consultations fee is
$90.00 and Follow-Up Phone Consultation is $60.00.
Laboratory tests: $ 100 - $ 400.00 depending on the amount of tests needed.
Q: Can I use recent laboratory tests performed in another doctor's office?
A: Yes. Very often patients bring saliva, blood or urine hormone tests performed
recently (not older than 3 months) by a recognized laboratory and we can use
those.
Q: Do you prescribe HGH, Testosterone or any other hormones for Body
Building?
A: No. Any hormone used for body-building reasons is against our beliefs of
normal or physiologic hormone replacement. Body Builders tend to use
larger-than-recommended doses. For anti-aging and health preservation
purposes you can achieve great health and physique with physiologic hormone
replacement, proper diet and exercise.
