Male Sex Hormones (testosterone, estrogens, DHT and progesterone)

Testosterone (and its metabolites or end-products) is responsible for the masculine
appearance of the body. It is also responsible for many other functions including brain
function, bone and muscle and the heart. With age (normal aging) the production of
testosterone declines to about less than half after the age of 75. This decline is
responsible for decreased strength, lack of mental capacity, fatigue, decrease libido
and weaker or absent erections, a process called "Andropause". With the increasing
life expectancy of modern times and more people looking for healthier bodies and
minds after age 80, testosterone replacement therapy has an increasingly important
role.
DHEA supplements can increase the body testosterone. If a deficiency is
detected by the laboratory test,  this hormone can be prescribed in several forms,
including injections, creams and implants (see
Testosterone Pellets Implant).

Normal aging in males is also associated with an increase in
Estrogen. Several
causes are responsible for that, including an increase in the conversion of
testosterone (facilitated by the enzyme aromatase) to estradiol. High estrogen has
been associated with prostate swelling, heart disease and breast swelling/tenderness.
The increase in estradiol creates an imbalance in the Testosterone/Estrogen balance
which is one of the reason that testosterone replacement in a low testosterone male
does not produce the expected improvement in symptoms. Excessive estrogen
(estradiol) can be corrected by avoiding alcohol, caffeine, reducing body fat or by
Aromatase inhibitors like Arimidex (Anastrozole).

DHT (dihydrotestosterone) is derived from testosterone (by the action of the
enzyme 5-alpha-reductase) and is 10 times more potent. DHT is responsible for the
virile body, beard, moustache, body and pubic hair, deep voice and erections.
Sometimes an increased DHT is associated with male pattern hair loss and excessive
body hair. This can worsen after testosterone replacement therapy is initiated, and
most of the times is corrected by stopping caffeine and alcohol, loosing body weight
(fat), limiting meat or prescribing 5-alpha-reductase inhibitors like Finasteride
(Propecia).

Progesterone is also an important component of this "aging hormone orchestra".
Progesterone is responsible for helping to keep estradiol and DHT levels low, is a
precursor of cortisol and other androgens, and stimulates spermatozoid
motility/fertility. Low progesterone is usually associated with high estrogens. Bio-
identical progesterone by oral route is the recommended when replacement is
needed.
